PRIMARY ACCOUNTABILITY:
WCMS seeks an empathetic, values-aligned Bilingual Community Health Worker (CHW) who is passionate about supporting the health and well-being of underserved patients. The CHW supports care coordination for patients enrolled in the Project Access (PA) donated services program by ensuring enrolled patients can fully access PA program benefits and by serving as a vital link between patients, healthcare providers, and community resources. The CHW also provides social support, informal coaching and patient education, and assists with external resource navigation.

Pay: $22 – $24 per hour – full-time (40 hours per week)
QUALITIES FOR SUCCESS IN THIS POSITION:
- Strong written and verbal communication in both English and Spanish
- Able to organize and appropriately “juggle” multiple activities on a daily basis
- Ability to communicate clearly and professionally with a wide variety of people, including in emotionally escalated situations
- Prioritization of patients’ dignity, agency and privacy through a culturally aware lens
- Extensive knowledge of healthcare, social service and community-based resources, and sensitivity to patient needs in these settings
- Ability to learn and follow established processes while demonstrating flexibility in novel situations (“problem-solving orientation”)
- Embrace of a team-based approach mixed with solid personal accountability
DUTIES (essential job functions not inclusive of all duties a CHW may perform):
- Communicate with patients about benefits and responsibilities under the program
- Assist patients with scheduling and keeping healthcare appointments
- Arrange patient transportation and interpreter services as needed and support patients with utilizing these services (including light “digital navigation” as needed)
- Identify and connect patients to community-based resources and services
- Support Diabetes Intervention Program, including some food delivery as needed
- Create culturally informed health education materials on topics such as preventive care, medical self-advocacy, digital literacy, and managing chronic conditions
- Support and develop initiatives to collect and document patient feedback and input
- Assist patients with applying for other healthcare discount programs as applicable
- Occasional use of personal vehicle, with standard mileage reimbursement
FOREIGN LANGUAGE SKILLS: Bilingual fluency in Spanish/English required
TECHNOLOGY SKILLS: Proficient in MS Office (Word, Excel, and Outlook). Comfort with running database searches. Ability to use and model use of mobile technology (text messaging, apps).
WORK ENVIRONMENT: WCMS operates with a hybrid workplace. Employee is required to work in-office (South Asheville) two days per week and to live within the sixteen county WCMS service area.
